All Spirometry Candidates who are undertaking the MCQ exam must have a nominated invigilator to invigilate their MCQ examination.

This might be a Head of Service, Deputy Head, GP, Senior Nurse, Medic, Supervisor or other senior member of staff. An Invigilator cannot be someone who is currently undertaking the ARTP Spirometry Assessment themselves.

Invigilators will need to complete this consent form each time an MCQ examination is undertaken, which confirms that no other materials were used in the examination and that the examination was completed by the named candidate. Without this completed form, the MCQ cannot go ahead and will be cancelled if not received before the booked exam date.

The following is to be completed by the invigilator no later than 48 hours before the MCQ examination date
